Notes:

1. Front page has a different style
2. Table of contents and foreword are not needed
3. Footer should be different
4. Sections shouldn't be numbered and there should be a separate page for each section, with the centered title at the beginning of the page. E.g. title before the main table should be "List of the national authorities designated to assign ITU-T Recommendation T.35 terminal provider codes", without header.
5. Empty rows don't have a proper height
6. Rendered tables are using auto-width, while their width is fixed in the original documents
