Split guidance for Referral and PEM CDA messages #5

We currently use identical message formats for both Referral and PEM transactions (using the CopyRecipient mode of a referral message for PEM).

We should consider whether we need to separate these message specifications to recognise that the type of information and use cases for PEM messages differ to those for referral messages.

NOTE: This feeds into the text section headings issue also.
