Program actual participating health facilities into enrollment form in Tracker #146

Currently, for demonstration purposes, we have placeholder health facilities (n = 4) in the enrollment form in tracker. The Senior Foreign Investigators have provided us with a list of health facilities from Nigeria and Kenya that will most likely participate in the eHBB/mHBS study (below). This list is subject to potential modification at the eHBB/mHBS Investigators' meeting, in Nairobi, on 8-9 September.

At a point in time determined by @sunbiz (before or after the Nairobi and Busia mHBS trainings and demos?) we should program these health facilities into enrollment form in Tracker.
