Adapt existing mHBS user guide to reflect latest version of mHBS #152

@BhavaniSingh , I know you are working on this, I wanted to open an issue so that this very critical task will not falll off our radar screen. We need to make a mHBS v2.0 user/training guide for the Nairobi project launch.

Please have a draft for you and I to review together on Sept 4, we will incorporate final edits, and get printed in time for our training of project staff on Sept 6. Thanks!

We need to update the user guide based on the updated forms, and the user feedback from the survey.

The updated guide is now into 3 modules:
