@mui/material example #41

The integration examples refer to @material-ui package, since version 5 the package has been renamed and a few breaking changes occurred. Please provide more up to date examples using @mui/material.

The new one is https://codesandbox.io/s/materialui-v5-n44lz if you want to contribute you could create a PR adding on the Readme example section named CodeSandbox Examples the link as well as in the following file https://github.com/iusehooks/usetheform/blob/master/docs/index.mdx .
