How can I remove the addin #39

I install the xla file, but now I want to move the file to another place. How can I change it or uninstall it?

Also, I am using Office 365. It always suggests that, the macro is forbidden because it is not trusted, how can I fix this?

just go to add in and uncheck it. Then browse and add new one
