This PR is meant to:

re-instate Ray Plante as editor since this will be a 1.1 revision
add a new pointer to the NVO starting cone specification (location to be fixed possibly)
removing the todo labeling (already changed into proper issues in the repo)

It will close issues #22 and #23

@jd-au @msdemlei I'll implement both your comments before merging, hoping it won't remove your approval.

re-adding the role_diagram.pdf means a change in .gitignore, but that's fine. On the NVO specification preservation we can chat about at TCG, I suppose.

I hopefully solved your comments in a different way than initially thought.

Now both .html are not in this repo: link to 1.03 goes straight to the IVOA DocRepo, NVO document is linked from an attachment to ConeSearch IVOA wiki topic. I changed the README and the footnote in the .tex accordingly. This means there's no reason for the extra folder in the git repo, that consequently looks now cleaner.

I restored the role_diagram.

I also updated the Makefile date of the WD to be ready for a proper WD release.

(In my silliness I committed/pushed too much, so I need a new review and approval)
