MAP entry modal - provide tool tips for key items

It appears that many novices are confused right at the entry modal. The first item asks for contributor name, but this is quite unclear. So most important is to provide immediate help here. For these keys for key terms, in addition to the usual mouse-over tool tips, "?" icons with the same text should be placed immediately to the right of term:

• routes
• contributed
• Contributor Name
• internet service provider (ISP)

I have added the tooltips. I have some objections to further increasing the visual clutter of this page with a bunch of question marks. We don't have any precedent for this type of visual/behaviour flow (Eg it doesn't appear anywhere else). Why these and only these terms? I get that these are some of the more confusing ones, but I think we should talk this through more
