Fixed sample threat model in README.md #168

Hi,

I have tried to build a threat model diagram from the provided sample script at Creating a Threat Model in README.md, but there were some missing imports and 2 lines got mixed up (a Dataflow attribute has been changed before its initialization).

Should be fixed with these changes :)
