Misc edits for implied-shape array, RANK clause, and glossary #118

Implementing the UTIs we put in during the last meeting. Also a couple other changes that are relevant.

Note my note!

Note: The change to C864 above, along with the normative text change,
      also allow RANK to work with a PARAMETER constant, not only a
      deferred constant, because they say "named constant" instead
      of "deferred constant". I think that's fine!

(I didn't modify the change to C864 in this PR, it's right above one of the changed portions.)
