Add link to QT feedback survey to the QT confirmation email

Background The JAC would like QT feedback surveys to have better response rates. In order to do so, we want to give multiple opportunities to candidates to fulfil their survey. Currently the link to the survey is only displayed on the confirmation screen at the end of the test. Candidates perhaps would prefer to fill out these surveys in their own time, some may not have the time or might not feel like doing so immediately following a test. In order to give candidates further opportunity to fill out their survey, it should be added to the email they receive upon QT completion.

User Story As a QT participant, i would like a link to the feedback survey in my confirmation email so that i can complete this survey at a time other than immediately after taking the test.

Feature(s) Description Add the feedback survey to the notify email template, passing in the link added by admins.

Acceptance Criteria

• ensure link appears within the received automated email.
