Revenue Summary Issue for Dr. Shares #94

1. In old practice whenever we need to create doctors monthly share, we can 
search procedures from department with doctor name filter. 

2. In current setting,
Daily Doctor Share Revenue Detailed,  
    we can select department and doctor in daily doctor share report for all wards and all type of patients but no record display.

Revenue Detailed by date with Share,
    we can select only department and doctor field is missing.
    after execution of report no data displayed.

so we selected "Revenue Detailed by date" to view whole month patient count for 
share calculation. after report execution procedures are not showing doctors 
names.
