Introduce a second "Mentoring Subject Area" field and a second "mentoring competency" field

What steps will reproduce the problem?
1.Under "Manage Training" click "mentoring" select "Add Session"

What is the expected output? 
Introduce a second "Mentoring Subject Area" field
Introduce a second "Mentoring Competency " field
What do you see instead?
There is only one field each for "Mentoring Subject Area" and "Mentoring 
Competency " fields
