After some additional testing, I have found the cause.
If you have the system ringer sound set to 'none', it causes the problem above,
although sometimes, the incoming csipsimple voip call will use the ringer
setting from the csipsimple config.
If I change the system ringtone to something different that 'none' and also
different than the csipsimple ringer setting and make calls to the csipsimple
line, I get the system ringer tone and not the csipsimple one.
I was setting the system to 'none' because I don't use the Cellular number
assigned to the phone by my carrier for incoming or outgoing calls at all, so I
silenced the ringer to eliminate rings from telemarketing calls.
So I hope this helps to isolate the issues. Strange that the odd call does
however use the csipsimple ringer setting, but not very often.
