CFIE is not working with or without ACA unless geometry is a sphere, but that is out of scope to fix in this issue so it will be done in a follow on. The machinery to solve the CFIE at a WS mode with or without ACA is all there, but will need further verification after fixing the CFIE bug.

This issue got out of hand and really turned into a large refactor as well.
