Initial thought is that Quan-Elixhauser is not based on ICD-9-CM (which has fewer definitions for HIV than the WHO system). Quan works in Canada, yet his comorbidity maps are used globally.
We should look into this discrepancy.
Current the package does not have ICD-9 (WHO) edition, although this is now not restricted in its distribution. Almost always, ICD-9-CM has more detail than WHO; this may be the only exception.
Yes, this is indeed because HIV in ICD-9-CM is a subset of WHO's ICD-9. Mostly ICD-9-CM is a superset, but this is an exception. The co-ormidity functions should catch it, though. See other similar issues: solution likely to be clarifying whether ICD-9 or ICD-9-CM is being used (at least if there is a discrepancy).

I would like to let you know that the ICD9 code “0449” is not defined or explained. However, the code is listed under Quan_Elix comorbidities for the category HIV.
