showing form not filled even after filling for ta preferences

What steps will reproduce the problem?
1. login as TA
2. Select any preferences and submit them
3. Go to application tab

What is the expected output? What do you see instead?
It should have shown the preferences we have filled.Instead it is showing form 
not filled

What version of the product are you using? On what operating system?

Please provide any additional information below.
Image 1 shows that form has been filled \. Image 2 shoes the error
