Selected Preferences are coming wrong

What steps will reproduce the problem?
1.Login as TA
2.Make some Preferences (Image 1)
3.Click on Save Form (Next page shows wrong preferences, Image 2)

What is the expected output? What do you see instead?
The expected output was to show the correct preferences of the TA. Instead it 
showed the first one, three times.

I believe if someone has already submitted their preferences, then the database 
is not getting updated on filling the preference again even if it says 
preferences saved successfully. Hence the error.

The form has already been filled by some other group member of yours. This is 
why the page is being shown. 
We can clear the database once again tomorrow, but this time ensure all of you 
(or atleast some of you) test it together as we won't be able to clear the 
wrongly entered values in the database every time.

I filled it only this way. Thats the error. I fill it once using TA login. I go 
back the page and fill it again, it shows this error.
