Partially failed decode H.264 stream, both DXVA and software

What's interesting - ffplay/PotPlayer/Microsoft DTV-DVD Video Decoder decode 
normal.

Seems fine to me, I also corrected the timestamp problem that caused choppy 
playback.

But failed if decoder connect to MEDIASUBTYPE_AVC1 type at LAVSplitter

I strongly advise against doing that. The option to do that may even disappear 
in the future.
