What steps will reproduce the problem?
1. using your test page, search for zip code 34471
2. I also checked the sqllite db and its also missing form there but zipcode
33122 works

What is the expected output? What do you see instead?
No Output, suggestive that the zipcode i am entering is not in the database
What version of the product are you using? On what operating system?
Mac + Win7
