Later in this document, section 2.2.3 refers to 11 as a 4-byte offset, not a 
3-byte offset.  From looking at the code and unit test, it appears that 4-byte 
is correct.

I also changed the heading of 2.2.3 to be consistent with 2.2.1 and 2.2.2 
("offset" instead of "offsets")
