Cannot add or remove a field in a consultation #351

What steps will reproduce the problem?
1. Create a consultation
2. Fill in 2 out of three fields
3. submit the the form

What is the expected output? What do you see instead?
The third field is mandatory, there is no way to submit it.
One need to use a fake third option to be able to submit the field.

There used to be a +/- button to remove or add a field.
