Dealer application form #15

Allow for dealers to apply for an account through an online application form.

Also will have links to printable PDF's for the account application forms for faxing in, and a printable credit card information form to fax in. SSL could be used to transmit the credit card information as well? but preferably over fax

if fax is the preferred method then i suggest we'll stick with fax.
