Discoverability of annotation create button #28

The button in the margin for creating annotations is not necessarily
obvious.  What I would really like to do is simply use the entire margin as
a create annotation button.  This is difficult because clicking on the page
removes the current selection (buttons are an exception - clicking a button
has no effect on the selection).

The whole margin is now the button in Marginalia 2.
