Support for more than 4 axes in Mission Planner from Joystick input #436

As far as I can tell the Mission Planner does not support more than 4 axes as 
inputs from a joystick. This appears to be because rather than binding Joystick 
axes to input channels you bind pre-defined input channels to axes.

What brought about this discovery was the desire to use the second throttle 
input on my joystick to control tilt on my camera mount.

Also support for the PoV hat button inputs would, I'm sure, be widely 
appreciated as well.

i will have a look at adding channels 1-8, so all functions should be possible
