Issue 404 does solve the removal of label issue but no Fields can be added afterward #407

After using the new RemoveAll in FieldSet, the fields and labels get 
removed fine.  The problem is that no new fields can be added after they 
have been removed.

Removed changes in FieldSet and added them as a 'beforedestroy' event in Field. 
 This 
way, they will be removed correctly for all type of forms.
