Please try another loader just to confirm it's a cios issue.

Well.... tried GX and that will not even load the game with v9 for me, just a
black screen.
When I load it in GX I hear my HDD click then spin up. About 10-15 secs later
it does the same but returns to HBC (else stays black screen). This happens
regardless of Emulation ON or OFF.
It loads fine in GX with v8 (I do not hear the HDD spin up at all when loading)
It's a Iomega drive, basically:

Never had any issues with it (he says)

No one reported any issue with games on real nand.
Are you sure you've set base 57?
Also please try with a different hdd or a pen drive.

OK :P
I've tried a 2TB WD drive and the same result with wiiflow - keeps trying to
load after joining enough players in lobby - the 'normal' multiplayer doesn't
get to game lobby, just loading screen after selecting multiplayer.
Do you think it's region related? I've just tried Mario Kart (PAL) and works -
Club Penguin (PAL) also went online and updated (I think I didn't have emunand
selected when I reported it not working last time, sorry)
I'll try and get the PAL version of Black Ops and see ...

I got it to load twice out of about 50 attempts with GX.
The first time I didn't have NAND set, so that bird with nice red lips (looks
like she can suck a golf ball through a hosepipe) started bleating numbers....
went back turned NAND on (nothing else) and it took me another 25 attempts to
get it started....
Guess what? It does the same as wiiflow X_x

hmmm... please try with gx dol for slot 249 after installing base 57 in that
slot.
And of course let gx launch black ops with cios 249.
I want to see what happens if the cios is not reloaded.

Makes no difference
reinstall d2x v9 > ios249 base 57 GX 249.dol - let it load as that with
NAND=FULL
Still when I connect to players and game tries to start the loading icon just
goes round and around and around.
I know it's going to work in v7/v8 because that loading icon begins to
'stutter' then it stops going around and game starts.

This is a known issue with the online update and a workaround exists.
Create a 1st profile an try to connect.
If it doesn't work then create a 2nd profile and try to connect with that.

I'm not sure, but I think the above answer is confused with something I said on
irc..
The problem still persists, but I got a PAL version and tried that (that's
where the confusion is - online update?)
The above answer relates to getting the online update into emunand save etc...
That works, but the same problem is there in PAL as this issue relates to - I
can connect to players in game lobby, but as soon as game tries to start (Game
starting in 3,2,1) I get black screen as opposed to cartoon screen. << with
PAL game
