unable to schedule an appointment

What steps will reproduce the problem?

Service and Providers combo boxes should load. They don't.

What version of the product are you using? On what operating system?

On the backend I am able to create users, customers, etc.
Now and again I would see the following:

Unable to connect to your database server using the provided settings.
