Added missing chip to BOM #2

The original BOM was missing the 74HCT138 chip at U2. While correcting this, I updated the BOM listing to a table format.

I updated the README with your changes. Rather than merge the pull request, I just updated the README by itself because the pull request also had some updates to the Kicad files that I wasn't sure about.

If you look at the file histories, you’ll note the KiCad files changes where all yours. I had to rebase my branch so that it would merge cleanly into master. I made no changes to KiCad.
