implement staff vs patient portal

• staff login page with master password set in config (TODO update this instead with email check in sheets)
• once staff is logged in, user_key passed in URL on all links to persist staff session
• headers updated to reflect whether in patient portal or staff portal
• staff logout fn returns w user_key=patient, default page of index
• profile scoped by staff vs patient (5 rows of patient data)
• registration while 'logged in' as staff returns staff view of patient data
• check in page for staff also allows staff view of patient data
