Links tagged in XML #15

Part of #10 but smaller scope. Links to other parts of the reference will be formatted:

<ref target='file_name'>link text</ref>

And external links will use normal HTML syntax, i.e.:

<a href='http://page.linked.to'>link text</a>

Actually, schema change. We'll just use:

<ref>file_name</ref>

And I'll have the generator script fill in names automatically, to keep things consistent / easy-to-update.
