Switch to ATX-style headers #1

Currently, headings use a generalization of the MediaWiki syntax (for an hn heading, surround the heading text with n = characters, with the relaxation that the trailing = characters can be the wrong number.

Replace this with Markdown/ATX style headings (for an hn heading, precede the heading line with n # characters).
