Respect preformatted text in preprocess_entry

Currently, the preprocess_entry subroutine will:

find link-like syntax inside preformatted blocks/inlines
perform link/signature rewriting inside preformatted inlines (including MathJax inlines)

Switch to a hashing scheme like render_entry uses in order to avoid this.
