Add ability to delete a server from the front-end #33

Is your feature request related to a problem? Please describe. Once servers are added there's no way to remove them without updating the database directly.

Describe the solution you'd like Provide a button or similar method to remove a server from showing up in Uffizi. With Uffizi querying plex.tv for servers, it might also be needed to have a way to ignore servers from plex.tv.

Describe alternatives you've considered Deleting the server directly from the table in the database.
