Unclear that sheet count starts at zero #10

It's unclear from the form and the wiki that the sheet count starts at
zero. Need to 
 * add a wiki entry explaining it
 * link to the wiki entry from the form

Alternatively, make the sheet count start at 1!

Changed so that sheet numbers start at 1
