Support :with-title option #6

Like in the patch in https://lists.gnu.org/archive/html/emacs-orgmode/2017-08/msg00553.html, documents with a nil :with-title option should not get the title added to the document.

There’s a failing test for this in https://github.com/jeffkreeftmeijer/ox-md-title.el/commit/0d67c20be69f343bc711a90db5d684da14de2e1d, in the with-title branch.
