Clean lists of columns in various places #535

Right now, no form title in many places where mass lists of columns exist, and you have to go off the column heading which can be hard to read/decipher. We need to group things by form, etc. But also, all forms in a relationship are coming in, regardless of whether they are directly connected to the main form (see for example the list of columns in a LOE screen settings on the Entries tab - for the "advanceview" settings). Forms in relationship with no direct connection to the main form should be excluded in such situations!

This was improved by 9d478d29b856530a6c45657eb306595ca2f4fa0f but more smarts are needed.
