As per CP-800 on the CSD profile (after its correction and clarifications, see https://groups.google.com/forum/#!topic/ihe-csd-implementors/8k2s6iqZKL4) we need to extract the epid/ellid as follows:

The entiryID will ALWAYS be a UUID formatted as a URN, the URN stuff should be parsed out and the uuid used as the epid/elid
The assigning authority should be set to the required value in record/sourceDirectory
