Implement tapered dosing for substanceAdministrations. Basically:

Might have one substanceAdministration with multiple frequencies specified by effectiveTime
Might have different doseQuantities which are represented by nested substanceAdministrations This will just require a modification to the MedicationsEntryProcessorImpl to support multiple frequencies. Additionally orders for split dosing need to be handled or not supported.
