Update/add a MedicationProcessor to support combination doses. This refers to GI cocktails and shall be represented as a drug name of "GI Cocktail" with subordinate substance administrations handling each of the components of the custom mixture.

Handled basic case. GI cocktails are not fully supported but combination medications are implemented. Needs more testing (see testing issues). Incorporated in : 2247d202abd319009d5b71d2113d51fb3312b2e4
