Duplicate allergies and problems should be detected on the condition and shouldn't create multiple list entries on each CDA.

This appears to be caused by a bigger problem with OpenMRS. Calling voidObs() and then subsequently loading the obs via getObs() does not preserve the voided flag at all. Even though in the database the voided column is set to 1 and I see the void reason being populated.
