• added and cooled all code from survey form project done in the admission trials
• updated read me file by adding attribution and acknowledgement.

Hi, kindly go through the html and css files again, I can see some places you can add comments 🤗 For example the student has a div wrapping the header tag, it can be anything just go through the project requirements and add inline comments that can improve the code 🤝
