Trivial change (already did on a local build), but I would suggest that the server type list box be replaced instead with a text box that allows the user to set the server url manually. For those of us on orgs that enforce MyDomain urls, the base plugin is unusable unfortunately. I would tend to think that most people interested in setting up CI with Salesforce are probably fairly aware of their appropriate server url. I suppose it is a tad bit of extra typing.
