Attempt to organize an initial release

This is a proposal first draft release, organized into:

• remedy.css for primary remedies we recommend as a baseline
• reminders.css for remedies to consider depending on the needs of your project
• <special>.css (e.g. quotes.css) for more specialized and optional remedies

I'm also making some attempt to resolve issues that have been discussed at length. Hopefully this PR can help us settle on a direction, but we can always re-open issues after merging if needed.

Once we have a solid draft, we can use individual PRs more consistently to add, update, or remove remedies. This PR should mainly serve as a proposal for how things are organized.

Resolves #46, and also:

• remedy.css
  • closes #2 - keep nested h1's consistent
  • closes #14 - block/responsive replaced elements
• optional reminders.css (closes #49)
  • closes #7, closes #9 - spacious line-heights
  • closes #11 - jump to end of animations, and turn off transitions/fixed-backgrounds

* add a top vertical alignment for the label for textarea elements?
* remove bullet points next to radio and checkboxes?

This PR isn't trying to be final or complete - just organized, working towards a pre-release. Can you file issues to discuss those additions (or see if they belong in existing issues)?
