Arbin SQL Server support #159

Step 1. support reading raw-data directly from the SQL server Step 2. recipe on setting up a "copy-based" solution (step 3. meta data extraction, step 4. implement into Batch, can wait to relies 0.5)

Hard-coded sql query implemented (@Ozzstein). Works, but needs more testing. Need to implement options to the sql query still. And also documentation and best practices for how to work on a copy of the original db.
