Basic Outright waivers #18

This creates a page for outright waivers. We mark playesr who are on waivers and have a new model for claims.

Things I've yet to do:

[x] Ability to post someone to outright waivers
[ ] give an indication to the current user of their current claims (grey out boxes)
[ ] Prevent non-team affiliated users from sending a claim request (also greyed out) ~- [ ] Admin page to manage players on waivers/claims~
[ ] Management command to execute claims
