PDF seems to be offline, but I have saved a copy. Let me know if anyone needs it! Hopefully, they will continue posting updated data...

The document went offline and we can't find a new version on https://www.quebec.ca/sante .

This data is now available from these links (only in PDF format though it seems). Last update on Dec 30, but not sure how often these documents are updated.
