Duplicate for Medical History #4

Duplicate is not detected with adding medical history, this may cause confusion to user due to multiple same history

Input:

First: pt ma 1 m/diabetes

Second: pt ma 1 m/diabetes

Expected: reject second input as it has been recorded

Actual: save exactly the same medical history

Medical records contains the exact same thing for every input and is added on the same day. This should not happen as the records are duplicated.
