(I'll assume you're using the native mac client and not the multiplatform
client, is that right?)
Can you get logs from the mac application after this happens?
(You can use the console at Applications > Utilities > Console, then search for
MacDroidNotifier)

I also have the same issue but on Windows XP. I can pair my Droid w/ Froyo
with the desktop program, but nothing will pop up when I receive a phone call
or SMS. The log shows that notifications for 'ring' and 'ping' are received,
but there is no pop-up. I am also using the native client, and no growl.

I'm also having trouble seeing any popup messages. Using MultiDroid 0.5.1 on
Windows XP, from a Nexus One on Froyo (2.2.1).
On the desktop settings, I have set the notification to be copied to the
clipboard to verify that is is received, but I am unable to see any popup
messages on screen.

You are receiving notifications without errors but you might have a problem
with Windows not showing up popup balloons, take a look at this issue:

With the help of the link in the comment above, I solved my problem with the
notifications not popping up. Thanks very much.
Though the link in comment 8 leads to a workaround for Windows 7 and me using
Windows XP, I figuered out that I blocked the Windows balloon pop-ups with
XP-Antispy. Once removed this setting and after rebooted my system, I was able
to get the notifications.

It's also working for me now. In Windows XP, you need to set EnableBalloonTips
to 1(DWORD). This is found under:
HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Explorer\Advanced
After logging off and back on, I reinstalled MutliDroidNotifier and it's all
working now.
