Red5Phone module needs to show more useful information #232

Right now when you click the headphones icon to connect to a MeetMe
conference through the Red5Phone integration, there is no useful feedback.
 Here are some things that are needed:

1 - show something that says "connecting..."
2 - show something that says failed if the call is rejected by the Asterisk
dialplan (or other failure)
3 - show something that gives the user a way to "hang up" or turn off this
feature after connecting (they may decide to dial in by phone instead)

1)Echo test button.
2)Mute button
3)Volume slid bars

Also, could we allow them to choose which microphone on their computer to use?  
I'm
not sure if that's available in the Flash API.  I have a beta tester reporting 
this:

I connected a client to the conference and even though I have a USB headset 
plugged
in, it wants to use the built in microphone on my HP notebook. No way that I 
can see
to control that.

We also had that issue but disabled the on-board microphone and speakers to get 
around 
the problem. I would suspect that some sort of mini applet would have to be 
written to 
do the same thing before flash got involved in the media stream.

Merged into issue #122 - preflight checklist.  

This pre-flight checklist could give the user lots of feedback and assistance 
and not complicate the interface of BigBlueButton itself.

I'm un-merging this because I really don't feel that #122 covers most of what 
this issue is describing.  Much of this issue is about in-conference controls 
for voice.  The voice controls right now for a user are non-existent.  A user 
should be able to mute himself and see the status of his connection.  Think 
about the webcam control.  You don't just hit a button and then start streaming 
the webcam without feedback.  You can see what you're sending.  You can close 
it.  Et cetera.  Voice controls need a similar functionality.  There should at 
least be a status indicator, a mute button, and a disconnect button.

We have all three now in 0.81-RC3.  

The webcam icon works as the status icon (it shows a green checkmark when 
connected)
There is a mute button always visible in the toolbar
The user can click the webcam icon to disconnect.
